Should you wish to apply for admission, please download one copy each of the following forms, and have them filled out properly and submitted to the Guidance Center, along with the other requirements:

• Application Form (to be filled out by the applicant or applicant's parent/guardian)
• Recommendation Form (to be filled out by the applicant's school adviser or guidance counselor)
• Health Certificate (to be filled out by any licensed medical doctor after a medical check up)
• Customer Feedback Form
